No, the referral form must be completed and signed by a licensed physician or nurse practitioner.
What conditions can be referred using the form?
The form facilitates referrals for conditions such as Depression, Anxiety, PTSD, Mood Disorders, among others. Please specify any additional conditions using DSM-5 guidelines.
